Chapter Change Form

Please fill in the old and new chapter and university names you wish to update. Note: a change of chapter or other fields may require CFA or CSI Headquarters approval before the change is made.

You may update your address, password, subscriptions, and member status (Entry Level (Master's) Student, Doctoral Level Student, Professional Counselor) directly within your CSI Member Dashboard. Simply log into your Member Profile (www.csi-net.org < Sign In < CSI Member Login) and make desired changes within your CSI Member Dashboard.

We recommend using the Mozilla Firefox browser (vs. Google Chrome, Internet Explorer, etc.) when completing and submitting forms.

You will see your chapter affiliation updated in your member profile typically within 5-7 business days.

Please be advised that submitting a Chapter Change Form does not change the Chapter Faculty Advisors for either chapter. Chapter Faculty Advisor changes may only be submitted through the CFA Change Form.
