*Reactivating chapters must be located in nationally accredited programs or those who have administrative approval to pursue and achieve national accreditation (CACREP and/or CORE) within the next 5 years from the date of the reactivation application. (sample letter) If these conditions cannot be met within the five years from the date of the activation of the chapter, such chapters will be inactive and ineligible to initiate members or otherwise function as an active chapter.

__________ Letter of administrative approval to pursue and achieve national accreditation (CACREP and/or CORE) within the
next 5 years
__________ Submit a copy of current chapter bylaws (sample chapter bylaws may be downloaded and include a completed Chapter By-Laws Review Rubric
to attach to your bylaws.
